I am a clinical nurse educator in the Intensive Care Unit on the journey to pursue my DNP in nursing leadership. While rounding in the ICU, I have noticed that many patients are recurring to the hospital because of unaddressed needs, or social determinants. As a result, addressing social determinants have become part of our rounding process and I have become passionate about population health, especially how frontline staff can play a role.
