Jennifer Polakowski, MPA, serves as the Community Liaison for NJAC which supports cultivating relationships with community organizations to match volunteer nurses with community projects that address the social determinants of health with a goal to improve the health and well-being of a community. Jennifer also is the Deputy Director of the New Jersey Nursing Initiative overseeing NJNI’s programs which focus on getting baccalaureate level nursing programs to incorporate concepts of population health into their curricula and the development of a Population Health Seminar Series in NJ for nursing faculty and clinicians. Jennifer has co-authored on this work published in Nurse Leader and presented this work at ENRS 2019.
